- Provide safe and effective patient care services, while promoting safe and effective access to medication therapy.
- Provide patient-centered care through interaction and facilitating effective communication with patients, caregivers, and stakeholders.
- Participate in the identification and implementation of medication-related interventions for a patient population (population health management). Conduct pracrice advancement projects.
- Demonstrate leadership skills that contribute to departmental and/or organizational excellence in the advancement of pharmacy services.
- Demonstrate leadership skills that foster personal growth and professional engagement.
- Provide effective medication and practice-related education.
- Provide professional and practice-related training to meet learners' educational needs.
- Complete all additional activities required and outlined by individual Pharmacy Residency Program
- Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy. Completion of an ASHP accredited PGY1 residency.
- Experience gained through completion of a PGY1 residency program. Required
- LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
- LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
- CRT-Basic Life Support (BLS) - AHA American Heart Association 180 Days required Or
- CRT-Basic Life Support (BLS) - ARC American Red Cross 180 Days required Or
- CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required Or
- CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required
