About the role

Essential Job Duties:

  1. Discharge Planning – Attends interdisciplinary rounds with Nursing and Medical Staff to assist in planning post-hospital care in nursing homes, home care or alternate placements.
  2. Counseling – Counsels, as needed, patients and families regarding their illness.
  3. Assessment and Referral – Coordinates referrals to community agencies in helping meet the needs of patients (e.g. transportation, meals on wheels, financial assistance, vocational rehabilitation, Veterans’ services, home health care and skilled care.
  4. Documentation – Documents on all cases in a timely and accurate fashion according to Social Service and Hospital policy.
  5. Child Abuse and Neglect – Counsels and refers in coordination medical and nursing staff, all cases of child abuse and neglect as appropriate.
  6. Adult Abuse and Neglect – Counsels and refers in coordination with medical and nursing staff cases of adult abuse and neglect.
  7. Adoptions – Coordinates adoption cases as needed, providing counseling and coordination of details.
  8. Prioritizes as necessary in order to meet important deadlines.
  9. Performance Improvement – Participates in P.I. projects as requested by Social Service Manager.

Secondary Job Duties That May be Reassigned:

  1. Financial—Counsels patients and families regarding disability, Medicare, Medicaid and other Financial resources as available.
  2. Community Education and Special Projects – Works with Social Service Manager in development Of Community Education Activities and special
  3. Completes other duties or special projects as assigned.

Job Specifications:

  • Education: Master’s Degree in Social Work. LSW or LCSW.
  • Experience: Two years of hospital Social Work experience preferred
