We will be a big FAN of yours too if, at a minimum, you have:

• A High School diploma or GED equivalent and be at least 18 years old,
• One (1) year of relevant experience working with Individuals with intellectual or developmental disabilities, behavioral challenges, dual diagnoses, or in the care of the medically fragile,
• A current driver’s license and California automobile insurance coverage for driving your personal vehicle to and from various community locations either independently or with other team members and/or Individuals, and if a company vehicle is available for use, one must also be 21 years of age, and 
• Ability to read, write and speak English.  Bilingual in English/Spanish is preferred.

• You will provide assistance in the initial and on-going assessment of participants and work with assigned Individuals, their family and “circle of support” (e.g., social service agencies, health care providers, etc.) to ensure that behaviorally challenged and/or medically fragile Individuals can develop and/or maintain positive experiences while participating in community social programs and daily routines. You will accomplish this by identifying, documenting, and providing first-hand training and developmental support across a broad spectrum of activities, e.g., in the areas of physical health, personal grooming, mobility, job and personal skills training, effective communications, socialization and coping skills and other customized interventions based
