Description
Work shifts will be between 0700-1630, Monday through Friday. Normal work scheduleswill be 9-hour shifts, to include a 1-hour non-paid lunch each day, not to exceed 40 hours
per week or 80 hours per two weeks.
HCW Qualifications (Contract PWS 5.2; 5.2.3; 5.2.4; 5.2.6.4.4):
• Certifications in addition to Basic Life Support (5.2.3): None
• Degree/Education (5.2.4):
o Bachelor of Science (B.S.) in Nursing
o Graduate from a college or university accredited by National League for Nursing
Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing
Education (CCNE)
• Experience:
o A minimum of 2 years as a RN with full-time experience in utilization
management/utilization review or in broad-based clinical nursing and clinical case
management working with adults, children, families, seniors and groups within the last 3
years. Have completed 30 hours of continuing education in nursing within the last 3 years.
• Board Certification (5.2.6.4.4): None
• Licensure/Registration: Current, full, active, and unrestricted license to practice as a Registered
Nurse as required in the TO.

HCW Duties (5.1 Task 1): The duties for the HCW are as follows:
• Participate in all phases of the Case Management Program (CMP) and ensure that the
CMP meets established case management (CM) standards of care
• Provide nursing expertise about the CM process, including assessment, planning,
implementation, coordination, and monitoring. Identify opportunities for CM and identify
and integrate local CM processes
• Develop and implement local strategies using inpatient, outpatient, onsite and telephonic
CM
• Develop and implement tools to support case management, such as those used for patient
identification and patient assessment, clinical practice guidelines, algorithms, CM
