Job Summary:

Geisinger Population Health team is seeking an experienced Nurse Practitioner or Physician Assistant to join our team in this fully remote position.

Job Duties:

What You'll Do:

• Focus on several key areas in the management of complex patients who are in the Accountable Care Organization (ACO)

• Combination of virtual Annual Wellness Visits, virtual Transitions of Care on complex patients, and virtual acute visits

• Work alongside a team of case managers, social workers, and community health workers as a partner with primary care providers

Requirements:

• Certified Nurse Practitioner or Physician Assistant who resides in Pennsylvania with an active Pennsylvania license

• Experience with telemedicine/virtual care is required

• Experience managing patients who are both medically and socially complex is required

• Exposure to and familiarity with an Accountable Care Organization and value-based care is preferred

• Ability to work both independently and as part of a collaborative team

• This is a fully remote position

• Full-time, Monday-Friday
