Job Description:

The Director of Clinical Programs and Value Based Care works under the direction of the Vice President of Clinical Quality and Value Based Care (Skilled Home Health) to ensure we have the right clinical programs with the outcomes that demonstrate the quality, and the value of the programs provided. The Director of Clinical Programs and Value Based Care is responsible for driving clinical excellence across our skilled home health operations. This role focuses on the design, implementation, and evaluation of strategic clinical initiatives that enhance patient outcomes, reduce hospitalizations, and ensure regulatory compliance. The Director will lead efforts to develop innovative clinical programs, standardize best practices, and support long-term improvements in care delivery processes.

As an Elara Caring Director of Clinical Programs and Value Based Care, you’ll contribute to our success in the following ways:

• Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Skilled Home Health team.
• Design and oversee evidence-based clinical programs tailored to the home health population (e.g., chronic disease management, fall prevention, palliative care).
• Outcomes focus on acute care potentially preventable hospitalizations programs and coordinates with operational leaders to prevent unnecessary PPH, in support of Value Based Purchasing Metrics and to meet/exceed expectations of our clients and referral partners as indicated by top performing TPS, star rating and NPS scores.
• Works with training and development/education and quality departments to achieve organizational goals

What is Required?

• Graduate of an accredited Diploma, School of Nursing. Masters Preferred
• Current State license as a registered nurse
• Minimum 8 years of experience as a registered nurse performing duties as a quality and operations leader
• Demonstrated success in operations and quality, performance improvement programs in a home health care or related organization
• Home Health OASIS and Coding Certification preferred
• Valid driver’s license and insurance
• Reliable transportation to perform job tasks
• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lb

You will report to the VP Clinical Quality and Value Based Care.
