Advanced Practice Provider Supervisor

Position Summary: 

 The Advanced Practice Provider (APP) Supervisor position will be held by an experienced APP or physician who serves as a primary care provider at Elevated Community Health. The job description for Elevated Community Health primary care provider is a separate document, and duties associated with that position are not repeated in this job description for APP Supervisor.

Under the general direction of the Chief Medical Officer (CMO), the APP Supervisor will support Elevated Community Health's mission and vision by providing strong service leadership for all medical provider services. The APP Supervisor will ensure the highest quality medical care, health education, and preventive service possible.
