About the role

Overview

Function as professional Registered Nurse to provide outpatient nursing care and assistance to our providers in Infectious Diseases.  Must be able to independently perform clinical and administrative duties.  Coordinate patient care by; reporting test results, assessment of patient status, educational needs, patient education and follow-up care. 

Responsibilities

Contribute to the well-being of the patients by serving as a positive role model and demonstrating a professional, caring attitude, and behavior towards coworkers and patients. 

 

Review, work, and complete tasks assigned to you and other team tasks.  This will include responding to patient messages, prescription refills, lab results, referrals, home health agencies, medication prior authorizations, patient forms and follow up from physicians in a professional and timely manner 

 

Notify patient of test/lab results and answer any questions regarding those results and any physician orders.  Assist patients with medication questions and clarification dosing. 

 

Appropriately and timely document all nursing intervention, including phone conversations into the patient’s electronic health record.   

 

Provide back -up to the providers in the clinic as necessary, triaging patients by obtaining vital signs, reconciling medication and allergy list, determining chief complaint. 

 

Patient education, discharge medication reconciliation, drug assistance programs, nutrition and barriers to care 

 

Obtain lab/CT/MRI/Biopsy results from outside sources and scan into the patient’s electronic medical record.  This includes reviewing upcoming appointments and obtaining prior to the visit. 

 

Obtain prior authorization for medications, and coordinate care. 

 

Participate in staff meetings and educational lectures/sessions.  Lead some clinical educational luncheons for staff. 

 

Schedule outpatient procedures and tests, including Infusions. 

 

Recognize and assess health education needs of patients.  Provide ongoing Education and Teaching. 

 

Completes other duties as assigned 

Qualifications

Graduation from NLN Accredited School of Nursing with RN required, BSN preferred. Must have a valid Virginia license as a Professional Registered Nurse.

 

Prior clinical experience is preferred.
