The Inpatient Hospice & Palliative Care team is seeking a compassionate Advanced Practice Provider (Nurse Practitioner/Physician Assistant) to join them providing consults to adult patients who present to the Emergency Department.
Corewell Health emergency departments experience high volumes of patients with serious illnesses who require complex symptom management and end-of-life care discussions often leading to prolonged ED stays, suboptimal patient experiences, undesired and inappropriate hospital admissions, high cost of care, reduced patient and provider satisfaction and overall reduced quality of care.
As a Hospice & Palliative Care APP who partners with the ED you will work with our interdisciplinary team by approaching patients to provide health literacy, ascertain goals and then collaborate to find an appropriate disposition. The ED embedded Palliative Care team can manage symptoms, facilitate goals-of-care discussions, and coordinate with other healthcare providers to improve patient outcomes, reduce hospital admissions, and enhance overall patient and family and provider satisfaction.

Opportunity Highlights:
- Functions in the role of Palliative Care APP to coordinate comprehensive care delivery for patients in the ED.
- Ensures that operations are in compliance with professional standards, applicable laws, accreditation, compliance/regulatory requirements and that organizational goals are being followed. Practices in accordance with the Michigan Public Health Code.
- Delivers care in a manner that is culturally sensitive and preserves patient autonomy, dignity and rights. Acts as a patient advocate, crossing multidisciplinary lines to meet patient needs
- Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Corewell Health. Acts as a consult in area of expertise.
- Documents in the medical record and dictates information as appropriate.
Qualifications:
Nurse Practitioner:
- Required Master's Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master’s, post-master’s or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
- Previous experience functioning in a collaborative role as a Nurse Practitioner preferred
- LIC-Nurse Practitioner (NP) - State of Michigan
- CRT-Basic Life Support (BLS)
- CRT- Adv Cardiac Life Support (ACLS)
Physician Assistant:
- Required Master's Degree Graduate of an accredited Physician Assistant educational program
- LIC-Physician Assistant - State of Michigan
- CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants
- CRT-Basic Life Support (BLS)
- CRT- Adv Cardiac Life Support (ACLS)
