Position Summary:

The Inpatient Unit Social Services Coordinator is a key position to the overall clinical program for each patient. The position assesses, develops the care plan with input from the treatment team, the position does clinical services in the form of meeting with patients in both individual and group settings, and helps plan discharge. The position works under the supervision of the Director of Social Work. The position expects a high degree of professionalism and work ethic. Documentation is expected to be timely and thorough. The position will protect the private health information of the IPU patients they interact with. The pace of the Unit will fluctuate but can become very busy, so the position needs to be able to manage pressure and be able to adapt to changing conditions on a regular basis.

 

Essential Duties and Responsibilities:

  • Complete an intake (biopsychosocial assessment) within 24 hours of client admission to PUC and 24 hours of client admission to IPU.
  • Provides orientation to the IPU and PUC. Educates patients on their rights under ISDH regulations, grievance process, provides expectations, and explains rules and regulations.
  • Receives clinical supervision from the assigned Social Services Director or designee.
  • Intervenes with escalated individuals using therapeutic techniques for de-escalation and perform Advanced Physical Skills when needed to ensure the individual’s safety and the safety of others.
  • Complete a treatment plan (ISP) which accurately reflects the client’s needs and family and/or natural support/involvement to aid in their ability to meet discharge criteria with goals and objectives that are time limited and behaviorally stated.
  • Complete and document authorized screening tools for each client
  • Provide individual and group therapeutic services including family/support, to be completed while on the PUC/IPU and ensure clients are receiving appropriate care based on their individual needs.
  • Provide evidence-based treatment through Illness Management and Recovery (IMR), as needed.
  • Maintain regular contact with PUC/IPU client’s current outpatient clinician to ensure continuity of care.
  • Maintain client contact schedule and document client contacts using appropriate PUC/IPU codes in EHR.
  • Document timely and accurate information, in accordance with Center policies which reflect client progress based on the appropriate therapeutic modality. Therapeutic services will be time limited and specific to the presenting problem(s) of each assigned consumer.
  • Report client progress to the treatment team during rounds.
  • Participate in treatment team meetings with others involved in the client treatment plan (internally and externally).
  • Provide education, both to the client, and the client’s significant other/family about the client’s illness.
  • Provide information to clients and their families about available community resources.
  • Provide individual, group, family therapeutic services, and case management as needed to clients based on the assessed need.
