The Student Health Care Center (SHCC) at the University of Florida is seeking a full-time Clinical Programs Coordinator II. In this role, you will enhance our case management system to ensure seamless care for our patients.
The Clinical Programs Coordinator II (Coordinator) is critical in coordinating student healthcare, ensuring continuity of care, and providing emergency interventions when necessary. This position involves close collaboration with the Student Health Care Center (SHCC) providers to monitor and manage a comprehensive case coordination system, offering follow-up and referral services, particularly for high-risk patients. The coordinator is responsible for substantiating services through meticulous documentation and contributes to program development, outreach, and training initiatives. Additionally, the role requires excellent organizational, communication, and problem-solving skills to develop and maintain collaborative relationships across campus and within the community. The coordinator is also tasked with enhancing individual and organizational productivity and fostering a psychologically healthy workplace.
Coordination of Student Support Services:
- Assist students with social services needs including, but not limited to, referral to community counselors, psychiatry providers and medical specialists. Facilitate health insurance applications and connect students with various University of Florida (UF) resources.
- Coordinate the sharing of clinical information among those involved in the student's care, including the student health psychiatry (SHP) provider, Counseling and Wellness Center (CWC) therapist, or community psychiatric provider/therapist. Follow up to ensure students have received appropriate services.
Collaboration with Psychiatric Care Programs:
- Collaborate CM services with those hospitals, intensive outpatient programs, and partial hospitalization programs to provide psychiatric care to UF students to determine appropriate discharge planning.
- Assist with community referrals, the academic withdrawal process when indicated, and meet with students who may be suitable for SHP services. Assess student stability and manage appropriate referrals.
- Oversee one or more client care and treatment programs, ensuring the effective delivery of services and adherence to established policies.
Data Collection and Documentation:
- Substantiate CM services rendered by collecting data, documenting in the medical record, maintaining accurate, current departmental statistical records, and submitting other necessary reports or surveys as needed. Coordinate closely with third-party vendor Mantra Health for increased psychiatry services.
Communication and Advocacy:
- Act as a liaison between the program and healthcare professionals, coordinating medical, psychiatric, and psychological services.
- Work with SHCC Marketing to develop and execute plans for marketing and promoting mental health clinical services to the UF student population.
Continuous Assessment and Improvement:
- Conduct ongoing assessments to evaluate the effectiveness of services, including patient satisfaction surveys, needs assessments, and exploring opportunities for expanded services.
- Develop, recommend, and implement policies and procedures to enhance client care and treatment programs.
- Work closely with the SHCC Quality officer to implement quality assurance programs, particularly related to our psychiatric program.
- Engage in continuous professional growth through participation in educational programs and workshops and reviewing current literature.
Typical Schedule:
- Monday through Friday, 08:00 AM to 05:00 PM
- 40 hours/week
