Essential Duties and Responsibilities

• Deliver therapeutic interventions designed by a supervising Physical Therapist.
• Provide strengthening, ROM, balance, and mobility training in the home setting.
• Track improvements, document visits, and communicate changes using an intuitive EMR.
• Teach exercises, safety techniques, and functional strategies to patients and caregivers.
• Partner with PTs, nurses, and other clinicians to ensure coordinated, high-quality care.

 Knowledge, Skills and Abilities

• Reliable, insured vehicle and valid driver's license.
• Strong communication skills and a patient-centered mindset.

 Education & Experience

• Home care experience preferred, but comprehensive onboarding is provided.
