Job Description
As a Lead Healthguide, you will provide direct guidance and leadership to the Healthguide team by serving as a subject matter expert in Guidehealth policies, procedures, workflows, documentation, practice communication, and patient engagement. Additionally, this role will develop, deliver, and evaluate training programs that assist Healthguides in implementing patient-centered, value-based care. This role reports directly to the Healthguide Supervisor and engages in cross-collaboration with Education, Compliance, Quality, and value-based care clinical leadership teams to ensure high-quality, team performance.

WHAT YOU’LL BE DOING

  • Ensuring current and newly hired Healthguide team members are equipped with the clinical knowledge, resources, and tools necessary to effectively coordinate care, improve patient health outcomes, meet performance measures, and adhere to Guidehealth policies and procedures.
  • Developing and maintaining training materials, job aids, workflows, and clinical documentation guidelines related to the Healthguide program.
  • In collaboration with the Healthguide Supervisor, delivering initial, ongoing, and refresher training to the Healthguide team on procedures, workflows, documentation, and care management programs (i.e., Transitions of Care, Compassionate Care, Congestive Heart Failure, Under 65, Condition Management Program, and future programs).
  • Serving as a resource for new hires during their transition period, answering clinical and workflow questions.
  • Providing real-time feedback, support, and coaching to ensure competency and confidence in new team members.
  • Monitoring progress of new hires and report performance or support needs to the Healthguide Supervisor.
  • Conducting skill assessments and audits to identify team and individual performance achievements and gaps.
  • Monitoring and reassigning patients in the care management platform (e.g., Arcadia Analytics) as requested by the Healthguide Supervisor.
  • Coordinating electronic health record (EHR) access and EHR learning needs for Healthguide team members.
  • Supporting the Healthguide team as a subject matter expert in using established tools (e.g., FindHelp, 211, Auxa) to locate community resources for patients with identified social needs.
  • Participating in team huddles and ongoing quality improvement initiatives.
  • Modelling professional, evidence-based clinical practice and upholding Guidehealth’s values and standards.
  • Maintaining a caseload at approximately 50% to stay current with workflows and ensure clinical relevance.
  • Providing support to the design and implementation of Artificial Intelligence (AI) outreach campaigns for identified populations.
  • Working primarily remotely, Healthguides collaborate with the patient’s medical practice staff and Guidehealth’s Case Management team to manage a targeted population of patients.
  • Functioning independently and as a highly collaborative team member as an extension of medical practice.
  • Assisting the patient in achieving better health outcomes by addressing their questions and concerns, connecting them with the medical practice, partnering in implementing high-impact care plans and closing care gaps, escalating medical issues, and navigating them to the right resources, including assisting with referrals to specialty care and community-based organizations (CBOs).
  • Accurately documenting patients’ medical history and medical issues in the EHR.
  • Demonstrating understanding of the referral and prior authorization process.
  • Continuously building a trusting, professional working relationship with assigned medical practices and within the Guidehealth organization.
  • Implementing and meeting assigned Key Performance Indicators to produce a positive patient health outcome, while decreasing medical spend.
  • Utilizing effective communication modalities (calls, texts, portals, emails) to engage and schedule high-risk patients.
