The individual in this position is responsible for the consistent delivery of high quality physical therapyto patients receiving care with Crossing Rivers Health.
Essential Functions Statement(s)
- Evaluates and initiates patient treatment, ensuring quality care is delivered throughout the treatment process.
- Establishes goals of treatment, per individual basis, and monitors progression accordingly.
- Educates patients regarding their disabilities/injuries and provides information pertinent to their recovery/rehabilitation, answering questions as appropriate.
- Communicates with all appropriate health care practitioners regarding patient treatment and progress, as well as informing appropriate family members/caregivers of treatment and progress.
- Demonstrates the knowledge and competence specific to the following age categories: neonates, pediatrics, adolescents, adults and geriatrics.
- Assists patients in acquiring necessary equipment adaptations or necessary medical equipment to aid in their rehabilitative process.
- Performs physical therapy treatment sessions, including but not limited to the following: stretching and strengthening exercises, gait training, advanced gait activities, balance exercises, transfer training, and staff/family/patient education.
- Maintains proper certifications, re-certifying as necessary; learns new techniques in Physical Therapy as available.
- Assists in the guidance and mentorship of students who perform clinical rotation with Crossing Rivers Health.
- Provides Physical Therapy services to agencies that Crossing Rivers Health has contractual agreements with.
- Complies with federal and state physical therapy regulations and requirements as well as organizational policy and procedure.
- Maintains patient confidentiality under HIPAA and adheres to organizational policy.
- Performs other duties and responsibilities as deemed necessary to meet patient, departmental, and organizational goals.

Requirements
Certifications & Licenses: Must hold and maintain an active Physical Therapist License in the State of Wisconsin
Education: Doctor of Physical Therapy (DPT) preferred.
Experience:
- Two (2) years of clinical physical therapy experience preferred.
