Hennepin Healthcare has a 0.80 FTE (64 hours per two week pay period, full benefits) opening for an Ambulatory Care Pharmacist in our HHS Ambulatory Care/MTM Pharmacy Department. This position is in person and will work day shifts, no weekends.
This position will support our MTM program by serving in an existing clinic or may serve in an expansion clinic, depending on departmental needs. HHS MTM services are delivered in both primary care and specialty care, and this role will likely involve a combination of these 2 practice settings. This pharmacist will be responsible for providing Comprehensive Medication Management (CMM) in all clinic settings. This pharmacist also will be credentialed and privileged through the Credentialing department at HHS.
SUMMARY
Works collaboratively with the patient and care team to ensure medications are indicated, effective, safe, and patients are adherent with therapy. Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy and ambulatory clinic. Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information and literature. Plans and organizes work schedule of supportive personnel (ex. interns, students) and pharmacists, as required.
RESPONSIBILITIES
- Provide Medication Therapy Management and/or disease state management to clinic patients
- Critically evaluate a patient’s medication profile to ensure that each medication is indicated, effective, safe, and that the patient is adherent with therapy
- Completes timely documentation of activities within the electronic medical record as appropriate for specific areas of responsibility
- Responsible for maintaining accurate and proper patient medication profiles, monitoring to ensure the proper choice of medication, dosage form, strength, length of therapy and interaction potential of the prescribed medication
- Provide medication information to patients, nurses, physicians or other health care practitioners
- Be able to critically evaluate and interpret medical information when providing specific information to physicians or providers
- Performs all responsibilities/duties required by the pharmacy as defined by the scope of service particular to each clinic to ensure that the unique nature of the client is addressed
- May precept Resident or PharmD student rotations
- Maintain current knowledge of pharmacotherapy, pathophysiology, and procedures in area of practice
- During patient encounter, responsible for interpretation of provider medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies
- During patient encounter, based on an assessment of the patient's specific medical needs, making interventions with the medical staff and nursing staff to facilitate optimal drug therapy and facilitate the provision of other pharmacy services
- Keeps up to date with competencies of both the pharmacy and those required in the clinic of practice
- May administer vaccinations, as applicable
- May work under Collaborative Practice and protocols as authorized by providers
