Job Summary

Summary Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor
(CRC ) provides direct patient care and participates in therapeutic milieu within the scope of defined practice.

Essential Functions
-Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
-Provides and supervises community activities according to needs.
-This may include driving & transporting patients to and from activities in hospital vehicles.
-Remains awake and alert, at all times, while on the job.
-Assists patients in maintaining and keeping various appointments.
-Clear, concise, and timely documentation and shift notes.
-Establishes relationships with patients that are helpful and maintains professional boundaries.
-Participates in patient rounds, status checks, or teams per program guidelines.
-Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
-Works effectively to manage the therapeutic milieu.
-Provides patients with direction and supervision as needed.
-Follows established hospital & program policies, procedures, and protocols.
-Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
-Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
-Encourage patients in their activities of daily living.
-Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
-Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
-Attends and participates in rounds and case conferences when appropriate.
-Physical ability for participation in patient activities.
-Other duties as assigned


 

Qualifications

Education High School Diploma or Equivalent required and Bachelor's Degree Psychology preferred or Bachelor's Degree Social Work preferred or Bachelor's Degree Health & Human Services preferred or Associate's Degree Related Field of Study preferred Can this role accept experience in lieu of a degree? No Licenses and Credentials Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health preferred Experience Experience working in a human services, residential, behavioral health, or allied health setting 1-2 years required and Experience working with individuals with mental illness, substance use disorders, or developmental disabilities 1-2 years required Knowledge, Skills and Abilities - Basic knowledge of behavioral health conditions and support strategies. - Strong interpersonal and communication skills, with the ability to maintain professional boundaries. - Ability to manage time, multitask, and respond to urgent needs calmly and effectively. - Commitment to person-centered care, equity, and cultural competence. - Familiarity with safety protocols and documentation standards in healthcare or social service environments.
