Amazing Care Home Health offers specialized therapy services that differ from other home health providers. At AmazingCare we give you the freedom to build a weekly schedule as you see fit with as many or as few visits as you would like alongside your preferred days of the week.

We are seeking a Physical Therapy Assistant to join our team in the Logan area. A home health physical therapist assistant (PTA) will implement treatment plans to assist clients of all ages with cognitive, neurological, physical, and/or sensory functioning by developing and administering physical therapy services in the home and community settings.

Key Responsibilities:

    • Coordinates and collaborates with the Physical Therapist and the Rehab Director to ensure integration of the therapeutic program in the plan of care.
    • Guides and teaches the patient in techniques, exercises, or precautions designed to improve range of motion, strength, balance, coordination, endurance, or functional ability in accordance with instruction from the supervising Physical Therapist as approved by the physician.
    • Observes, records, and reports the patient's reaction to treatment and any changes in the patient's condition or treatment. Documents the extent of the patient's involvement in the decision affecting his/her care.
    • Participates in patient case conferences to coordinate the patient's plan of care and promote positive therapeutic outcome.
    • Identify patient's needs and exhibit compassion, caring, and empathy to individual differences
    • Complete documentation to support the delivery of physical therapy services

Pay: $40.00 - $50.00 per visit, (Mileage Provided)

Mileage provided for commuting between patient visits

Schedule:

Part-Time Days ,10-20 visits per week

Requirements

    • Active Physical Therapy Assistant License (Utah)
    • Active CPR Certification (America Heart Association)
    • Valid driver's license and auto insurance
    • Experience supporting a mixed population (pediatric/adult)
    • Previous EMR/Charting experience
    • 1 year of clinical experience (preferred)
    • Previous Home Health experience (preferred)
    • Knowledge of physiology best practices and standards
    • Hands on experience with therapeutic exercises, functional training and physical modalities
    • Strong interpersonal/communication skills
