Department: Hospice
Shift/schedule: Full Time (40 hrs/wk)
GENERAL SUMMARY
Works under the supervision of the Nurse Manager of Hospice Outpatient. The RN's primary job function is to identify patient/family needs and provide supportive care in accordance with the attending physician's order, the Interdisciplinary Plan of Care, and the Hospice concept and philosophy of care by rendering professional skilled nursing care in the home and/or home-like setting. Performs other duties as assigned.
QUALIFICATIONS
Education:
• High School Diploma or successful completion of an equivalent High School Exam Required
• Associate's Degree in Nursing required
Licensure:
• Licensed to practice as an RN in Ohio required
• BLS within 90 days of hire required
• BMVL required (Must maintain eligibility to drive as determined by SOMC insurance carriers)
Experience:
• Two years as a Registered Nurse with experience working with terminally ill patients in a home environment preferred.
JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS
The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.
1. Assumes primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process.
2. Communicates and collaborates with physicians and other team members in developing and revising the Plan of Care.
3. In conjunction with the physician and other team members, evaluates patient's response to Plan of Care and updates the Plan of Care according to policy and procedures.
4. Provides appropriate support at the time of death and through the periodic of bereavement.
5. Initiates and participates in discharge planning process as indicated by policy and procedures.
6. Participates in the 24-hour, 7 day a week on-call nursing coverage for patients/families as appropriate.
7. Supervises ancillary personnel as appropriate.
8. Maintains up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
9. Maintains professional standards by participating in team meetings, in-services, performance improvement activities, etc.
10. Pre-cleans, decontaminates and transports contaminated instruments according to policy
11. Performs intermediate-level disinfection and/ or high level disinfection per the Instructions for Use (IFU) by the manufacture. This includes ensuring the scopes or probes used within the department are cleaned, disinfected, maintained and stored per the IFU’s
