LPN, Compliance, Education, and Quality Assistant

POSITION SUMMARY:

The Hospice of Baton Rouge is seeking a compassionate and skilled Quality Assurance Performance Improvement LPN to join our dedicated team. The primary responsibility of this position is to assist in maintaining compliance with all policies and procedures as required by LDH and CMS regulations and CHAP standards.

JOB FUNCTIONS:

• Assist Director of Compliance, Education and Quality with implementing and monitoring agency-wide compliance with THBR policies and procedures, LDH and CMS regulations and CHAP standards.
• Collect quality measures, analyze data, and present compliance deficiencies and trends to the QAPI Committee and managers.
• Interact with all interdisciplinary team members and provide insight to help formulate and update the care plan.
• Maintain accurate and current records, reports, and forms per documentation policies.
• Anticipate patient/family needs and utilize the interdisciplinary team and community resources appropriately.
• Teach the patient and family regarding the plan of care, interventions, aligned with their own goals, and the implications of the diagnosis and prognosis.  Utilize medications, DME, supplies, and non-pharmacological interventions.
• Act as a liaison between the hospice program, the patient’s primary physician, and any other involved providers.
• Record, perform audit, and review occurrence of adverse patient events (falls, infections, pressure ulcers) and quality of documentation of high-risk, problem-prone areas. 
• Conduct audit of admission technical documents for compliance with federal, state, and CHAP standards, pre-billing. 

Education:

• Diploma in Practical Nursing.

Licensure:

• Current Louisiana license from an accredited school of Practical Nursing.
• Current CPR Certification

Experience:

• Minimum three years of recent varied work experience as a professional nurse, and preferably one of the three years in an acute, oncology, home health and/or hospice setting.
• Knowledge of Medicare/Medicaid regulations and guidelines.

Skills:

• Ability to work independently as well as on an Interdisciplinary Team.
• Demonstrated skill in nursing practice, ability to cope with emotional stress and tolerance of individual lifestyles
• Must have good organizational skills and ability to prioritize effectively.
• Knowledge in use of computers, copiers, and other office equipment
• Good communication skills and techniques
• Supportive and sensitive to needs of patients, families and physicians, as well as staff and volunteers

Technology:

• Proficiency in electronic health records (EHR) and other healthcare software.

Other Requirements: 

• Must have own transportation and provide proof of liability insurance which meets state requirements.
• Must have good communication skills and techniques with facilities, patients, families, office staff, and physicians.
