No Weekends, Flexible HoursReporting to the Director of Operations, the Nurse Navigator – Outpatient Neurology will serve as advocate, educator, facilitator, and guide for patients who are moving through the treatment process at Abington Hospital – Jefferson Health (AH)
Job Description
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Facilitate patient care throughout various stages of treatment through consultation with patients, coordination of services, and communication with physicians and other healthcare providers to offer comprehensive, patient centered care.
- Provides education to patients, family and care givers regarding disease processes, treatment options and potential side effects. Help patients prepare a list of questions and concerns to raise with physicians/healthcare providers prior to office visits. Assist patients, family and care givers with treatment decision making; develop and use education aids as appropriate. Provide community outreach in the form of community education.
- Responsible for business development by actively soliciting and recording feedback from physicians and healthcare providers and develop recommendations for improving operations to better meet the needs of physicians and the patients who they serve. Meet with physicians that are potential referral sources for the program to provide information about the program offerings.
- Obtains pre certifications and/or pre authorizations for medications/procedures.
- Performs other duties as assigned.
