Hospice care nurse under job summary: The hospice case manager will be responsible for all aspect of care, including coordination of care with the hospice medical director, social worker, chaplain, and home health aid. Working as a team is essential part of the hospice program.

Job Responsibilities:

· Managing their care plans according to physicians’ instructions.

· Managing medication regimen for patients and teaching patient and family members key instructions for optimal comfort.

· Inspecting wounds, changing dressings, and handling personal grooming and hygiene.

· Listening to the concerns of family members and answering their questions.

· Educating caregivers and family on the ongoing care of the patient.

· Providing suggestions for improved comfort to physicians and family members of the patient.

· Keeping abreast of developments in healthcare and attending workshops and lectures as required.

· Collaborating with doctors and other healthcare professionals to develop the optimal plan of care for your patients

Attending Interdisciplinary group meetings as scheduled

Hospice Nurse Requirements:

· Active RN or BSN License

· Relevant license and certification.

Excellent observational and problem-solving skills.

· Nursing skills as defined as generally accepted standards of practice

· The ability to follow instructions but also to act independently if necessary.

· Excellent report writing skills.

· Active listening skills and empathy.

· Physical and mental strength.

· A valid driver’s license and reliable transportation, insurance required.

Education:

· Completion of an accredited Registered Nursing Program.

· Preferred, BSN.

Job Types: Full-time, Part-time

Pay: From $40.00 per hour
