Part-Time Community Outreach & Relations Coordinator

What You'll Do:
  • Connect with referral partners (hospitals, senior centers, discharge planners) to introduce our services.
  • Educate the community by leading sessions on dementia care, Parkinson’s care, and wellness for seniors.
  • Attend community events and represent Assisting Hands with professionalism and warmth.
  • Support families by performing in-home assessments and helping them create personalized care plans.
  • Track your progress using a CRM to follow up with leads, contacts, and outreach efforts.

What We're Looking For:
  • Experience in healthcare, homecare, senior services, or community outreach (LMSW, LPN, RN, BSW a plus).
  • Comfortable speaking in front of small groups and building new relationships.
  • Organized, self-motivated, and tech-savvy (familiar with MS Office, Google Drive, and CRM tools).
  • Passionate about helping others, especially seniors and their families.
