. Risk Management
- Manages the interdisciplinary facility-wide risk management program designed to address risk identification, reporting, analysis, investigation and reduction.
- Collects, evaluates, and distributes relevant data on client/staff injuries and other relevant risk management issues
- Oversee the review process of all facility incidents, including but not limited to:
- Reporting timeliness
- Medical equipment issues
- Medication errors
- Injuries
- Policy or procedure deviations
- Administrative issues
- Collaborates with clinical and administrative staff for risk mitigation when needed.
- Consults with department heads on client incidents and other risk related issues. Provides recommendations for, and monitors implementation of risk reduction and management strategies. Completes event investigation, root-cause analysis, and follow-up including Failure Mode and Effects Analysis (FMEA) for events.
- Integrates and coordinates risk management with all facility functions, policies and procedures in collaboration with appropriate department heads to include medical equipment biomedical regulations.
- Completes trend analysis of events, near misses and claims.
- Serves as liaison with insurance carriers. Coordinates insurance carrier surveys and plans of correction in response to survey findings.
- Prepares and submits documentation and reports in accordance with established timelines and in a manner determined to be appropriate by CMS, DBHDS, and OIG
- Annual risk management plan
- Quarterly risk management reports
- Individual reports of agency investigations (within 5 days to CMS)
- Follow-up reports (48 hour & 15 day investigations)
- Any other reports/documents requested
- Receives, evaluates, and processes all patient events and recommend action when necessary.
- Evaluates employee and visitor events and recommends action when necessary. Prepare a summary of events for Quality and Safety Committee.
- Submits in-depth reports of findings and recommendations to resolve operations deficiencies
- Ensures investigations and claims management information are identified, evaluated, and processed as to minimalize losses to HDMC
- Collaborates with appropriate parties to ensure that the facility maintains an inventory of all exposures requiring insurance coverage and that these needs are coordinated with DBHDS.
Provides monthly Risk Management employee training to new hires and on-going training to existing employees.
.Facility Investigator
• The investigator is assigned by the Facility Director/CEO.
• The investigator shall report to the Facility Director/CEO throughout the course of an investigation and consult with the Human Rights
