A Brief Overview
The Alliance Liaison enhances the continuity of patient care by providing collaboration between the hospital, physicians, hospice/home care, Home Medical Equipment Services and the patients and caregivers. The Liaison works with case management, social workers, physicians and others on the health care team to transition the patient to the post-acute care setting. This Liaison will be responsible for building and maintaining partnerships that enhance the post-acute care referrals to Alliance. The incumbent collaborates with others to develop and execute market strategies to targeted audiences, improve service offerings and implement initiatives that support improved patient care and growth in division sales. What you will do
- Participates as part of the Health Care Team to support a collaborative effort to facilitate quality patient care. Integrates and coordinates various aspects of service delivery, care facilitation, payer authorizations, service access, discharge and post-discharge activities and utilization review. Ensures documentation is complete by reviewing patient orders, verifiying insurance information, gathering clinical data and determining patient care needs.
- Identifies patients that meet criteria for Alliance Services. Facilitates the transfer of patients from hospital to home care/hospice services, consults with nursing, social services and other support services.
- Promotes and builds strong relationships with referring physicians and hospital staff in the Case Management Department.
- Provides information and education for health care team, physicians, hospital staff and patient/family, answers questions and requests and completes applicable reporting as required.
- Effectively communicates in writing and orally with patients/families and other health team members.
- Delivers and sets up portable medical and related equipment to necessary facilities as assigned. Ensures equipment is operating safely and proper training is completed.
- Demonstrates and promotes professional practice of all members of the nursing care team
Education/Formal Training Requirements
- Required - Associates Degree Nursing
- Preferred - Bachelor's Degree Nursing
Work Experience Requirements
- Required - Registered Nurse 3-5 years
- Preferred - Home care/hospice
Licenses and Certifications Requirements
- Required - Registered Nurse Tennessee - Tennessee Board of Nursing
- Required - BASIC LIFE SUPPORT - American Heart Association
- Preferred - Preferred: Certification in clinical specialty
Knowledge, Skills and Abilities
- Ability to serve as a patient advocate by making a connection with patients and families, listening to understand their needs, and honoring commitments.
- Skill and proficiency in communications and interpersonal relations.
- Skill and proficiency in applying highly technical principles, concepts and techniques, which are central to the professional discipline in Nursing.
- Analytical skills necessary to assess complex patient conditions effectively and to develop comprehensive plans of care.
- Synthesizes multiple sources of information to assist in the resolution of human responses to complex problems.
