Job Descriptions: 

This position will be housed in, or near, Washington County.  The Service Coordinator position is responsible for ensuring adherence to IDEA, Part C requirements regarding service coordination. Early intervention service coordination (defined in 34 CFR §303.34) is a mandated service under Part C of IDEA, provided at no cost to families. Service Coordinators are responsible for coordinating all necessary services to support families.  Service Coordinators assist families of infants and toddlers with disabilities by facilitating access to early intervention and other necessary services, including referrals and appointment scheduling; coordinating the delivery of these services, including educational, social, and medical needs; overseeing evaluations, assessments, and the development, review, and evaluation of IFSPs; helping families identify and connect with EIS providers and coordinating service delivery to ensure timeliness; conducting follow-up to ensure services meet needs and informing families of their rights and procedural safeguards under IDEA; managing funding sources for required services and facilitating transition plans to preschool or other services; assisting with projects for monitoring and family engagement teams as needed; and performing other assigned duties.

Summary

The Department of Human Services (DHS) Program Coordinator is responsible for developing and coordinating a statewide program. This position is governed by state and federal laws and agency/institution policy.

Functions

Supervises a professional and administrative support staff by interviewing and recommending for hire, training employees or arranging training opportunities, assigning and reviewing work and special projects, evaluating the performance of incumbents, and conducting staff meetings to address problems and obtain status reports. Establishes short and long-range goals, plans, and activities. Develops or participates in the development of operating policies and procedures, staffing, and program needs. Defines monitoring methods, monitors program or service delivery effectiveness and compliance, identifies problem areas, and initiates corrective action. Provides technical assistance to internal and external personnel by disseminating and interpreting laws, policies, and procedures Coordinates personnel and activities to identify program needs, problems, and progress and implement revisions or corrections. Monitors and approves program expenditures, prepares budget requests for program operations and develops and monitors provider contracts. Performs administrative duties by reviewing and analyzing statistical data, compiling Information, preparing reports and correspondence, and researching and recommending legislation, trends, and program options. Performs other duties as assigned.
