Behavioral Health Technician - BTG ReEntry (4605)

This individual provides direct care services (personal assistance and independent living skills training) and supervision of the patient in the own home. Terros Health is hiring an Full-Time Behavioral Health Technician for our Bridging the Gap ReEntry Program in Phoenix, AZ. 

High School Diploma or GED, as well as a Combination of Behavioral Health Education and Work Experience Totaling a Minimum of Three (3) Years

Full-time Schedule

40 Hours Per Week

Essential Duties:

• Responsible for direct face to face community-based services to assist client’s re-entering into the community from jail and prison. Ensures linkage into community services and resources
• Offers in-reach into jail, prison, sober living housing and treatment programs for the purpose of engaging member into BTG Re-Entry / TIP services. Orients potential clients to the BTG Re-Entry/TIP program.
• Consistently meets Service Delivery expectations including $2150 weekly productivity, Compliance, and timely filing (notes completed by midnight same day).
• Provides transportation for members as appropriate and as medically necessary, and determined by the Re-Entry/ TIP team, in personal vehicle or in Terros Health provided vehicle, when available.
• Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate. Coordinates care with a member’s family, behavioral and general medical providers, community resources, and other involved supports. Assists members with connection to physical health services, including Primary Care services and medical appointment follow up.
• Provides excellent customer service to members, internal and external provider agencies, state entities, or other stakeholders (e.g. probation officers, justice liaisons, advocates, family members, residential staff, hospital staff and vocational rehabilitation, etc.) as needed.
• Offers direct care wellness management and recovery strategies using strength-based engagement and psycho-social/educational /rehabilitation services as needed.
