About the role

Overview

Manages large and growing document library, maintains and updates web site content, organization and user interface. Provides coordination and tracking of all program activities, manages communications, event coordination, and messaging. Tracks results of initiatives and create reports for various leaders in support of the Knowledge Map goals.

Responsibilities

  • Maintains and manages computer databases to retain and monitor key department data, statistics, project plans.
  • Manages, tracks and updates web site and all web site content including a large and growing document library, user interface, user feedback.
  • Knowledge Map team workflow and resource allocation management includes: researching and implementing use of new and existing data/ telecommunications technology to enhance time management, effective communication, and document management.
  • Trains Knowledge Map staff on technology tools use, oversees implementation, disciplined application, updates and upgrades.
  • Monitors grant management activities, deliverables, deadlines and contract requirements, ensures grant compliance (current CMMI grant, future grants)
  • Organizes inputs from multiple contributors and synthesizes into documents including work plans and reports - Generates charts, graphs, narrative reports and power point presentations of collective work products.
  • Coordinates all marketing communications for internal and external KM products.
  • Builds, manages, and maintains document library of e-Consult /Enhanced Referrals templates, newsletters, reports, communications and other products related to this care model innovation effort.
  • Develops, implements and maintains satisfaction survey implementation and results management for diverse teams and committees.
  • Designs and implements communications and prepares and/or assists with the preparation and dissemination of other types of communications.
  • Meets with KM liaisons and coordinates cross site activities and resource sharing, which may require occasional travel to regional clinic sites.
  • Identifies and implements process improvement initiatives for delivering services more effectively and efficiently.
  • Acts as the primary point of contact and gatekeeper for the D-H Knowledge Map in creating effective relationships and managing contacts and relationships across all DH sites.
  • Ascertains and maintains datasets from external and internal sources, formats per internal guidelines and distributes to users, using BRIO and EPIC reports.
  • Works with the Administrative Director to prepare documents such as budget material & reports.
  • Performs other duties as required or assigned.
  • Qualifications

    • Associate’s degree with 5 years of operational experience or the equivalent experience required, preferably within a healthcare environment.
    • Must have excellent and assertive planning and organizational skills.
    • Must have demonstrated capabilities in developing a collaborative approach to program development, establishing policies and procedures, and ability to support project evaluation.
    • Independent thinker who is able to make decisions based on available information and data.
    • Proficiency in computer software, such as MS Office Suite including Word, MS Access, PowerPoint, Excel, Smart Sheets, etc. required.
    • Excellent written and verbal communication skills.
    • Enthusiasm for the entrepreneurial aspects of building a new program within a clinical organization is critical.
