Weekend Clinical Intake Coordinator (LVN / RN)
Full-Time Role; Saturday - Sunday & Monday - Wednesday

This is a unique opportunity for a clinically experienced LVN or RN to step into a high-impact operational role at the agency level. As our Weekend Clinical Intake Coordinator, you will serve as the central point of contact for our entire organization on Saturdays and Sundays, managing Kaiser referral intake, supporting field clinicians with clinical escalations, and ensuring our patients receive timely, seamless care coordination when our weekday leadership team is offline.
This role is ideal for a clinician with home health experience who thrives in a fast-paced, autonomous environment, communicates confidently with both clinical and operational partners, and can triage competing priorities with clarity and calm.
What You'll Do

- Triage incoming calls from the field, helping clinicians prioritize and navigate unexpected patient needs
- Coordinate open capacity across the field team in real time, matching available clinicians to urgent patient needs
- Serve as the first point of escalation for clinical questions, supporting staff in the same capacity a Clinical Supervisor would during the week
- Triage and respond to clinical escalation calls from field staff, patients, and Kaiser throughout the weekend
- Apply clinical judgment to de-escalate, redirect, or elevate issues as appropriate
- Maintain clear documentation of all escalations, actions taken, and outcomes
