Come join our growing team! The Care Team Hospice is seeking a Part-Time CNA to serve patients in Fort Bend, Brazoria, and Harris, TX. This role requires 12–16 hours of availability Monday through Friday, between 8:30 a.m. and 4:30 p.m.

Key Responsibilities
Under the direction and supervision of the RN, the CNA provides essential personal care and supportive services, including:

Assisting with bathing, grooming, mouth/skin/hair care, toileting, and ambulation
-
Supporting bed transfers and range-of-motion exercises as directed

Performing light household tasks necessary for the patient’s well-being

Preparing meals, including special diets when needed

Taking vital signs (temperature, pulse, respiration, blood pressure) when assigned by the RN
-
Assisting with shopping when no other resources are available

Offering emotional support, encouragement, and compassionate listening to patients and families
Documentation & Compliance

Document care provided in the clinical record within 48 hours of each visit

Participate in initial and ongoing (14-day) RN supervision and evaluations
-
Report changes in patient condition or family situation immediately to the RN and hospice office

Attend at least 12 hours of in-service education annually

Maintain current CNA certification and competency per state requirements
-
Adhere to OSHA infection control standards and all company policies
Qualifications
- Maintains the agency's mission, philosophy, and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
Regulatory Requirements
- Licensed or registered in the state in which currently practices
- Must pass a criminal background check & MVR check.
- Completed health statements acknowledging the ability to perform the duties of the position.
- Valid state drivers license.
- Proof of current automobile insurance.
- TB testing per agency policy
EDUCATION:
- High School Diploma or GED
- Graduate from an approved training and competency course.
