The Patient Care Coordinator functions as an integral member of the team and is the first point of contact for all people inquiring about the provider's practice. The primary role is to organize the practice's daily activities and paperwork. The Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals

Working Hours: A 40-hour work week, Monday - Friday, between the hours of 8:00am-5pm with the flexibility to stay later on clinic days

Primary Responsibilities:

  • Exhibits empathy, courtesy, competence, efficiency, and care
  • Schedules appointments and procedures, following standard guidelines
  • Uses multiple computer and phone systems to fulfill patient needs over the phone
  • Verifies patient information, while documenting in their Electronic Health Record (EPIC)
  • Advises patients of their responsibilities regarding insurance and referral information
  • Greet customers into the practice and provide instruction and/or direction as necessary on the phone
  • Schedule and coordinate all patient appointments as directed
  • Prepare all documentation and records with accuracy as directed or necessary for the patient's care
  • Meet or exceed the performance measures established by the Practice Management Department
  • Follow establish policy and procedures related to the position
  • Participate and contribute to a team-based environment
  • Maintain a team supportive attendance record
  • Obtain accurate and updated patient information, such as name, address, insurance information
  • Perform insurance verification on the date of service
  • Scheduling procedures in an organized and efficient manner
  • Knowledge of medical terminology and procedures and requirements
  • Ability to obtain authorizations for the medical procedures
  • Schedule appointments using approved scheduling guidelines
  • Assist coworkers as needed to assure smooth office operation and delivery of excellent service through teamwork
  • Demonstrate respect for patients and Optum staff by presenting a friendly, courteous manner at all times and maintaining patient confidentiality
  • Obtain patient signatures for required documents
  • File and maintain medical records
  • Confirms and schedule appointments
  • Answering incoming and outgoing telephone calls promptly and courteously,
  • Perform referral documentation promptly
  • Performs certain follow-up services for patients in a prompt and courteous manner, such as scheduling specialist appointments, scheduling follow-up appointments and obtaining copies lab results or specialist reports
  • Adhere to the standards identified via Sparq regarding Optum Employee Policies
  • Work cohesively with fellow employees to achieve specific team goals
  • Comfortable working in high pace environment 
  • Participate in planning, coordinating, and implementing care in conjunction with the multidisciplinary team in accordance with the Plan of Care
