About the role

Assesses and evaluates patients/families' social, psychosocial and environmental situations to determine patient needs. Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned

Role & Responsibilities:

  • Licensed Masters Social Work to provide services including discharge planning for acute care population. Inpatient and emergency room assignments.  
  • Experienced with acute inpatient, experience discharges planning acute care. Knowledgeable SNF and home care referrals. Proficient in biopsychosocial assessments. 
  • Participate in an innovative inpatient care program providing leadership to a multidisciplinary team. Geriatric experience.
  • Apply pertinent state and federal regulations regarding social work and discharge planning, familiarity with post-acute provider services, financial screening, and reporting requirements.

Requirements:

  • Masters in Social Work from an accredited school required. New York State License (LMSW) required. 
  • Experienced with acute inpatient, experience discharges planning acute care. Knowledgeable SNF and home care referrals. Proficient in biopsychosocial assessments. 
  • Knowledge of pertinent state and federal regulations regarding social work and discharge planning, familiarity with post-acute provider services, financial screening, and reporting requirements.
  • Minimum 1-2 years’ experience in an acute care hospital required.
  • Effective interpersonal, communication and organizational skills required, computer proficiency required. Ability to problem solves using critical thinking required
