Responsibilities

Conduct patient evaluations and develop personalized therapeutic treatment plans.
Provide hands-on therapy to patients in their homes.
Educate patients and their families on proper exercises and techniques to maximize physical function and prevent injury.
Monitor patient progress and adjust treatment plans as needed.
Collaborate with other healthcare professionals, including physicians and nurses, to deliver coordinated care.
Maintain timely and thorough documentation of patient progress and treatment plans.
Adhere to ethical and professional standards and ensure regulatory compliance.

Requirements

Physical Therapy degree from an accredited institution.
Current and valid state Physical Therapy license.
At least one year of experience as a PT in a home health setting is preferred.
CPR certification.
Ability to work independently, prioritize tasks, and manage time effectively.
Strong communication, interpersonal, and problem-solving skills.
Comfortable with technology, including electronic medical records and mobile devices.
Valid driver's license and reliable transportation.
