JOB SUMMARY:
The Clinical Supervisor/ Care Coordinator RN is responsible for providing quality triage and patient care utilizing strong assessment skills and point of care testing while practicing within the scope of practice defined by the State Board of Nursing.
The Clinical Supervisor/ Care Coordinator in collaboration with the Practice Manager and Clinicians, assures the provisions of quality care to patients, develops and maintains a mechanism to evaluate and improve the quality of care, works to improve care coordination and improve patient and customer satisfaction while providing a climate of collaboration between clinical staff and other disciplines. The Care Coordinator/Clinical Supervisor works closely with the clinicians to improve quality, patient satisfaction, and is involved in the ongoing development and growth of the practice and staff.
DEPT. SCOPE OF SERVICE:
The BMH-owned or managed physician practices provide medical services in various specialties and offices including OB/GYN, cardiology, orthopedics, general surgery, urology, podiatry, family practice and internal medicine. The practices are staffed by physicians and other licensed providers who are supported by clinical staff (RNs, LPNs or medical assistants) and clerical staff (receptionists). Hours vary by practice.
KEY RESPONSIBILITIES:
- Provides direct supervision of clinical staff
- Deliver comprehensive nursing care in a high-volume Cardiology and Neurology clinics, performing detailed assessments and triaging patients with acute and chronic cardiac conditions.
- Manage patients with cardiac implantable electronic devices (CIEDs), including pacemakers, ICDs, and CRT devices.
- Conducted in-clinic and remote device interrogations, evaluating battery status, lead integrity, and arrhythmia events, and collaborated with providers to optimize device programming.
- Assist with pre- and post-procedure care for device implantation, including patient preparation, wound care assessment, and complication monitoring.
- Educate patients and families on cardiovascular/neurology disease management, medications, device function, activity restrictions, and lifestyle modifications to improve outcomes.
- Ensured adherence to clinical protocols, safety standards, and regulatory requirements in both in-person and remote monitoring workflows.
- In collaboration with the clinicians and their office staff, reviews and coordinates care for patients in the CHF program that are identified as high risk and/or need care coordination with community resources with the goal of promoting efficiency and proactive care.
- Manage patients with cardiac implantable electronic devices (CIEDs), including pacemakers, ICDs, and CRT devices. Conduct in-clinic and remote device interrogations, evaluating battery status, lead integrity, and arrhythmia events, and collaborated with providers to optimize device programming.
- Acts as a vital liaison between patients, families, and healthcare providers, managing patient care plans, scheduling, and resources to ensure high-quality, efficient care.
Requirements
EDUCATION:
- Graduate of an RN accredited nursing program; BSN preferred.
- 3-5 years recent outpatient clinical nursing experience preferred
- Acute care experience preferred
- Previous experience with cardiac implantable electronic devices preferred
LICENSURE/CERTIFICATION(S):
- Active State of Vermont RN license
- Current Healthcare Provider CPR certification
