Behavioral Health Case Worker Lead

Position Purpose:
The purpose of this position is to work within an integrated system of care between primary care and mental health, ensuring that patients seeking medical services are screened for mental health conditions such as anxiety, depression and substance use and that their mental health needs are addressed.
MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:
- Ensure that new patients and as appropriate, returning patients complete required screening tools;
- Score behavioral health screeners and enter results into E.H.R across all clinics.
- Perform brief consultation with patients during medical visit for enrollment and or linkage in Behavioral health services.
- Work closely with medical providers and care teams to intervene with patients needing immediate consultation.
- Perform crisis intervention, safety planning, case management, referrals and follow-up as required;
- Develop a collaborative relationship with care teams, medical providers and other BH staff members.
- Participate in weekly and monthly staff meetings with care teams, BH department and supervisor.
- Complete all required documentation within 24 time period.
- Supervise 4 BH staff members and ensure medical floor coverage for BH department every week.
- Train all BH floor staff on all safety planning and crisis intervention.
- Present Bi yearly de-escalation and safety planning presentation to all Staff including medical providers.
- Provide weekly productivity for floor staff and meet one on one with staff regarding increasing enrollment numbers for BH services.
- Meet with Mental Health Director on productivity, floor coverage and care team issues on monthly basis.
- Manage After hours Crisis Calendar and provide coverage.
- Manager Care team schedules and assign different BH staff members to individual teams.
POSITION REQUIREMENTS AND QUALIFICATIONS:
- A Bachelor’s degree in Social Work or related field.
- Cardiopulmonary Resuscitation (CPR) Certification required.
- Three years of social work experience in a medical or mental health setting.
- Must be bilingual (English/Spanish) and bi-cultural.
- Strong interpersonal skills. Must be able to work within a team environment.
- Must have excellent written and verbal communication skills.
- Computer literacy in word processing and spreadsheet programs.
- Must be familiar and have the ability to work within an Electronic Health Records system.

- Ability to relate well with patients and exercise good customer service skills on the phone and in person.
- Ability to stand, sit, stoop, kneel, and bend.
- Ability to work with personal computer.
- Travel to and from clinic or meeting sites within geographic service area
DOE: $27.00 - $31.88
