JOB DESCRIPTION:
This position will work directly with the Pediatric and Adult Cystic Fibrosis Clinics.
The Outpatient Social Work Case Manager (SWCM) is a comprehensive role within the interdisciplinary care team, which delivers care to patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion, coordination and chronic disease management. The SWCM is responsible for assessment and intervention with any patient or family along the health care continuum where psychosocial and /or high-risk factors are part of or interfere with the patient’s treatments or health outcomes. SWCM utilizes advocacy skills, intervenes therapeutically and supportively, provides resources and referral information, and offers financial guidance and assistance, as needed. The Social Work Case Manager will provide implementation, sustainment, and support of all youth, young adult and adult screening as well as brief interventions and navigation to services through the SBIRT/SBIN Programs.
EDUCATION:
MSW from an accredited school of social work required. Licensure through the State of Vermont is preferred.
EXPERIENCE:
Greater than 1 year experience working in a human service organization, health care setting, or managed care system is required.
