Job Description
The hours for this position are Monday - Friday, 8:00 a.m. - 4:30 p.m. (however, start times could vary)
Saturday and Sunday coverage every 4 weeks.
The CNA - HHC reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The CNA HHC provides basic assistance to activities of daily living to patients and assists in maintenance of a safe and clean environment under the direction and supervision of the Registered Nurse. Enforces teaching regarding end of life care. Participates as an active member of the interdisciplinary team.
RESPONSIBILITIES:
- Demonstrates NM values in daily work and interactions.
- Presents a friendly, approachable, professional demeanor and appearance.
- Provides accurate information and timely updates to patients and customers. Addresses questions and concerns promptly, or identifies appropriate person and resources to do so. Provides directions or help to patients and customers with finding their way.
- Uses effective service recovery skills to solve problems or service breakdowns when they occur.
- Demonstrates teamwork by helping co-workers within and across departments. Communicates effectively with others, respects diverse opinions and styles, and acknowledges the assistance and contributions of others
- Uses organizational and unit/department resources efficiently.
- Manages work schedule efficiently, completing tasks and assignments on time.
- Contributes to opportunities and processes for continuous improvement.
- Participates in efforts to reduce costs, streamline work processes, improve and grow services we provide
- Performs basic patient care responsibilities, including vital signs, skin care, and personal care, considering needs specific to the standard of care for patient's age.
- Demonstrates ability to assist patient with Activities of Daily Living (ADL's) such as bathing, feeding, hair, skin and oral care. Maintains patient environment in a clean and neat manner.
- Follows standard precautions and CNS infection control policies and procedures consistently.
- Reapplies oxygen masks and cannulas without changing flow as directed; orally suctions patients.
- Manages and operates equipment safely and correctly, cleans equipment according to standards.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Protects the patients in the event of a medical/health care error and, as appropriate, assists in medical/health care error resolution.
- Performs other duties as assigned within the CNA scope of practice.
- Re-orientates patient, assists with ambulation/positioning and transports patients as needed.
- Meets current documentation standards and policies. Provides documentation that is timely, accurate, up to date, complete and legible.
Qualifications
Required:
- Current Nursing Assistant Certification
- Maintains CNA certification as required by Illinois State Certification Board. (requires continuous period of 24 months service to maintain certification)
Preferred:
- Home health or Hospice experience preferred
