Location: Hybrid Remote


The Medical Records Clerk is a full-time, hourly, non-exempt position. They prepare and release medical records as requested. They make sure the patient has completed an authorization for release form. Responds to requests for copies of patient’s medical records. Makes copies of medical records as requested by patients, attorneys, subpoenas, health care providers and for disability and life insurance applications. 


 

· Prepare/fill in FMLA and Disability paperwork.

· Responsible for managing, retrieving and responding to phone messages via Klara, Web EX, Microsoft Teams and/or other communication platforms

· Manages and assigns incoming faxes (Provider Flow, etc.) 

· Pulls chart from Mod Med (EMR)

· Keeps accurate records using Excel. 

· Empty Medical Records fax inbox Provider Flow 

· Check email for PSCC Operative Reports and upload it to chart and follow proper procedure

· Complete medical records requests.

· Respond to Subpoenas 

· Complete FMLA & Disability requests.

· Log all medical records and FMLA / Disability requests.

· Check voice care folder for pictures to be assigned to the patient’s chart. 

· Respond to/monitor Microsoft Team messages

· Communicate to the appropriate recipient in the manner designed for compliance

· Respond to/monitor intra mail

· Participate in staff meetings and medical records staff meetings.

· Complies with HIPAA regulations and maintains strict confidentiality of all patient information.

· To comply with all general office policies and procedures.

ATTENDANCE

Attendance each working day is an essential function of the job.


EDUCATION and/or EXPERIENCE 

High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Must have computer skills.


COMPUTER SKILLS

Have knowledge of basic computer skills. Experience in Adobe, Excel, and Microsoft Word. 


APMe/EHR SECURITY LEVEL

This job is listed under Medical Records security level. This position has browser rights to patient account information and appointments and can add referring physicians. HIPAA compliant.


LANGUAGE SKILLS 

Ability to read and comprehend simple instructions, short correspondence, and memos.  Ability to write simple correspondence.  Ability to effectively present information to patients and other employees of the organization.
