LIFE (Living Independence for the Elderly) is a Medicare/Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. Job Description

• Utilizes critical thinking skills and knowledge to ensure high-quality care, effective care planning, and risk management within the home care setting.
• Coordinates all home care services.
• Participates in educating the patient and family.
• Reviews and updates participants’ care plans.
• Provides supervision and oversight to PCAs and CNAs in the home care setting.

Qualifications

• Must be a LPN, PTA, or COTA currently licensed in the Commonwealth of Pennsylvania with one year experience with a frail or elderly population.
• Current Pennsylvania Driver’s License.
• Ability to effectively implement plans of care and coordinate care for participants.   
• Ability to work independently and as part of a team, under pressure, while maintaining cooperative working relationships.
