Outreach Clinical Coordinator

Are you a Registered Nurse looking to make a change while still making a big impact in the Hutt Valley community? This could be the role for you!

We're looking for an Outreach Clinical Coordinator to join our Hutt Valley team on a six-month fixed-term contract. While we ideally seek a full-time person to fill the position, we are willing to consider part-time options for the right candidate.

Our Clinical Outreach Coordinator plays a key role in ensuring whānau needing support are connected to the right services so they can access timely care.

Our ideal candidate will:

• Be a Registered Nurse with previous experience in the primary

health care sector

• Have worked in an environment where they helped identify barriers

to care and supported systems to remove those barriers

• Have a close eye for detail, the ability to juggle multiple priorities,

and think outside the box

• Be confident using the Microsoft suite
