Schedule:
Monday–Friday, 8:30 AM–5:00 PM
Every 12th weekend (Saturday and Sunday)
Territory: Crystal Lake, IL
Compensation:
Up to $97,000 annually
Final offer based on years of experience

What You’ll Do
- Serve as the primary nurse for assigned patients and families, including assessment, eligibility determination, admissions, care planning, implementation, and evaluation
- Assess physical, psychological, social, and spiritual needs to ensure holistic, patient-centered care
- Collaborate with physicians, interdisciplinary care team members, and community partners to coordinate optimal care
- Conduct regular patient visits to reassess needs and update care plans
- Perform clinical pronouncement of death and provide compassionate support to families
- Maintain timely, accurate patient documentation including charts, progress notes, and care plans
- Supervise and document care provided by Home Health Aides in compliance with federal, state, and company guidelines
Qualifications
- Graduate of an accredited nursing program
- Current Registered Nurse (RN) license in the state of practice
- Current CPR and First Aid certification
- Knowledge of quality assessment and assurance procedures
- Valid driver’s license and proof of insurance
