RN Hospice Case Manager
Position 1: Full-Time
Position 2: Full-Time On-Call (Monday-Friday)
Position 3: PRN
Position Summary
The RN Hospice Case Manager is responsible for coordinating and managing the plan of care for hospice patients across all care settings. This role provides comprehensive nursing assessment, care coordination, symptom management, patient and family education, and collaboration with the interdisciplinary team to ensure patients receive compassionate, high-quality end-of-life care.
The RN Case Manager serves as the primary clinical leader for assigned patients, promoting comfort, dignity, and quality of life while supporting patients and families through the hospice journey.
Key Responsibilities
Patient Care & Care Coordination
• Conduct comprehensive nursing assessments upon admission and throughout hospice services.
• Develop, implement, and update individualized plans of care based on patient needs and goals.
• Manage an assigned hospice caseload and coordinate services across the continuum of care.
• Monitor patient condition and respond appropriately to changes in clinical status.
• Provide symptom management and pain management interventions consistent with hospice philosophy and physician orders.
• Coordinate admissions, recertifications, transfers, discharges, and transitions in care.
• Participate in after-hours and on-call responsibilities, as assigned.
Interdisciplinary Team Collaboration
• Collaborate with physicians, nurse practitioners, hospice aides, social workers, chaplains, volunteers, and other team members to ensure coordinated patient care.
• Participate in Interdisciplinary Group (IDG) meetings and care planning discussions.
• Communicate changes in condition and care needs to appropriate team members.
Patient & Family Support
• Educate patients and families regarding disease progression, symptom management, medications, and available hospice services.
• Provide emotional support and guidance through end-of-life care decisions.
• Promote patient dignity, autonomy, and quality of life in accordance with hospice philosophy.
Documentation & Compliance
• Maintain accurate, timely, and complete clinical documentation consistent with hospice regulations and agency standards.
• Ensure documentation supports eligibility, recertification, care planning, and reimbursement requirements.
• Participate in quality assurance, performance improvement, and regulatory compliance initiatives.
• Support survey readiness and adherence to CMS, state, and federal hospice regulations.
Required Qualifications
• Current, active Registered Nurse (RN) license in good standing in Ohio.
• Minimum of two (2) years of nursing experience.
• Valid driver's license, reliable transportation, and automobile insurance.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, organization, and time management abilities.
• Ability to work independently while collaborating effectively within an interdisciplinary team.
Preferred Qualifications
• Previous hospice, home health, palliative care, oncology, or end-of-life care experience.
• Case management experience.
• Knowledge of CMS hospice Conditions of Participation.
• Experience with electronic medical record (EMR) systems.
• BSN preferred.
• CHPN certification or willingness to obtain certification.
Physical & Work Requirements
• Ability to travel throughout assigned service areas to provide patient visits.
• Ability to lift, transfer, reposition, and assist patients as needed.
• Ability to sit, stand, bend, walk, and drive for extended periods.
• Ability to perform clinical assessments and nursing interventions in patients' homes, facilities, and inpatient settings.
• Ability to use a computer, tablet, and EMR system for documentation and communication.
