Department: Home Health & Hospice

Reports To: Home Health and Hospice Manager

FLSA Status: Non-Exempt

Hours: 40/week

Shifts: Days, with rotating calls and weekends

WHAT YOU CAN DO FOR FCMC

Our Home Health and Hospice Clinical Manager roles are held by Registered Nurses who are responsible for smooth transitions as a patient goes from institutional care to home care. In everything from nursing duties to patient/family education or counseling, Clinical Managers offer compassionate care for their patients. More specifically, these responsibilities include:

  • Educating patients, caregivers, and family by teaching nursing principles and demonstrating nursing skills and techniques
  • Participating in the application and evaluation of the Plan of Care while collaborating with all involved therapies
  • Providing patient, caregiver, or family counseling, referring to MSW as needed
  • Initiating appropriate preventative and rehabilitative nursing procedures
  • Observing, evaluating, and reporting patients’ physical and emotional conditions to the physician, including completing OASIS assessments
  • Maintaining communication with the supervisor regarding daily visit schedules
  • Demonstrating commitment to continuing education and training, including attending staff meetings, educational programs, and professional conferences


Requirements

Qualifications:

  • Current licensure as a Registered Nurse in the State of Idaho
  • Current BLS; ACLS is optional
  • At least one year of direct patient care experience is preferred
  • Working knowledge of nursing, healthcare safety, state and federal regulations, and regulatory processes
  • Capable of supervising multiple aspects of patient care and individuals providing that care
  • Excellent written and verbal communication skills in English
  • Ability to manage, support, and work cooperatively with patients, individuals, and committees
  • Must have a valid driver’s license and available automobile and good operating condition


Essential Job Functions:

  • Regular and predictable attendance is required
  • Regularly required to stand; walk; use hands to finger, handle, or feel; reach with hand and arms; and occasionally stoop, kneel, crouch, crawl, or twist
  • Occasionally required to sit, climb, or balance
  • Occasionally required to operate foot controls and lifts
  • Frequently able to move or push up to 35 pounds and occasionally able to lift, carry, move, or push up to 100 pounds
  • Specific vision abilities include close, distance, color, and peripheral vision
  • Frequently required to drive a vehicle
  • Must comply with HIPAA and patient privacy
  • Must wear PPE as situations require
