Summary:
The Neuroscience Case Manager is a unique position and serves as a liaison between the Neuroscience Service and the nursing units as well as consultant and referring physicians. Reports to the Neuroscience Program Manager and is responsible for all aspects of the stroke program including stroke alert response, QI process, Core measures, Neuroendovascular program, throughput, Hub hospital support, community education and outreach. The Neuroscience Case Manager serves as a focal point for quality stroke care and to maintain Joint Commission Certification.
Job Requirements:
Graduate of an accredited school of professional nursing.
Current Ohio RN Licensure. Bachelor’s degree in nursing or related field.
Minimum five years of experience in critical care or neuroscience nursing.
Stroke certification preferred.
Demonstrates leadership skills and expertise in planning, organizing and articulating ideas and concepts.
Current CPR certification required.
Job Responsibilities:
Participates in activities to support and maintain Joint Commission Stroke Certification.
Demonstrates and maintains effective communication/collaboration skills with the multidisciplinary team. Acts as a liaison between patient, physician, family and health care team regarding plan of care and progress toward goals.
Facilitates neurology rounds/Neuroendovascular rounds with physicians, residents, medical students and mid - level staff. Maintains Neurology/Neuroendovascular patient list. Ensures discharge appointments are in place.
Schedules routine and emergent Neuroendovascular procedures. Assists with Neuroendovascular clinic appointments.
Functions as lead nurse for Neuroendovascular procedures including pre-procedure assessment, intra-procedure monitoring of patient and room environment and post-procedure recovery. Maintains Neuroendovascular nursing supply stock.
Facilitates the development of safe/effective plans of care for stroke patients. Daily rounding on stroke patient. Provides information to stroke patients and their families regarding stroke: expected course, plan of care, education.
Demonstrates technical and clinical competence in neuroscience nursing with an emphasis on stroke care. Serves as a clinical resource and role model for staff throughout organization.
Responds to stroke alerts and is part of rapid response and code blue team. Leads team in stroke alert process.
Maintains NIHSS certification.
Maintains 8 CEU’s per year in stroke education.
Performs formal and informal neuroscience education to staff.
Performs concurrent stroke core measure audits.
Participates in performance improvement activities in collaboration with the Neuroscience program manager. Carries out identified quality assurance and quality improvement initiatives.
Collects data and performs data entry for stroke registry (GWTG).
Assists with Telemedicine process assures appropriate/timely use (Plan for telemedicine).
Serves as a regional liaison/information resource for medical community for stroke protocol (Plan for telemedicine).
Assists with monthly mock stroke alerts with hub hospitals via telemedicine (Plan for telemedicine).
