Infant and Maternal Health Liaison (Full-Time)

POSITION OVERVIEW

The Infant and Maternal Health Liaison provides wraparound support to expectant mothers and parents of children age birth to five (5) through an array of services, including one on one support through case management, support groups, parenting classes, and community education or awareness activities. The Infant and Maternal Health Liaison collaborates with other professionals, community partners, and referral sources to support clients with relationship-focused, hope centered and trauma informed service delivery utilizing the Strengthening Families framework.

ESSENTIAL FUNCTIONS                                                            

• Promote and contribute to a work environment that encourages knowledge of, respect for, and development of skills to engage with those of other cultures, beliefs, abilities, and experiences.
• Demonstrate capacity for providing developmentally sensitive, hope centered, trauma informed relationship-focused evidence-based/informed services and supports. 
• Provide direct services to clients which may include referral, screening, assessment, service plan, progress notes or reports, and discharge services.
• Assess client needs and provide appropriate referral and linkage to resources as necessary.
• Participate in interdisciplinary team activities, meetings and educational opportunities for clients and families within the agency or through partnership sites.
• Facilitate and attend regular meetings, trainings, and evidence based educational classes to engage in intentional and collaborative problem solving and resource connection with clients.
• Promote and enhance the overall health and well-being of clients through services provided, evidenced by use of appropriate screenings and outcome measurements.
• Establish and maintain regular communication with clients and community partners as a support and resource.
• Responsible for implementing and facilitation of evidence based psychoeducational and prevention groups, classes, and development of support materials based on best practices.
• Accurately complete all record keeping, documentation, and reporting functions in a timely manner as required by program and/or agency policy utilizing the agency designated EHR or other designated reporting systems. 
• Maintain established productivity requirements for direct client care, classes, and activities.
• Establish effective community relationships for referral sources as well as build and update an ongoing resource and referral manual.
• Document evaluation and follow-up processes of participants, trainings, and speaking presentations to track performance outcomes, service outputs, and client focused needs.
• Participate in community engagement and outreach events to build program and service capacity in the communities served.
• Assist in efforts to raise awareness of program services components and recruit eligible clients.
• Assist in the coordination of events and partnerships related to serving agency clients.
• Provide consistent coverage and assistance to the front desk and reception areas to assist with direct client coordination.
