Assumes the day to day direct nursing care responsibilities for assigned residents. Implements nursing systems that produce quality outcomes for residents and demonstrate best practices. She/he assists all direct reports with the functions and activities of the nursing department and provides CNA oversight to ensure compliance with all nursing and regulatory requirements.

Essential Job Functions and Responsibilities:

• Performs all direct licensed duties with competency, accuracy, and timeliness. Takes full charge of all wounds, infections, weight loss, ADL issues, psychosocial concerns, etc. Directs any clinical issues that arise for any assigned residents. Completes all required documentation with competency, accuracy, and timeliness. Takes full charge of all documentation related to condition changes, incident reports with immediate statements, reportable events, inclusive of all care delivery forms necessary for any component of resident care.
• Takes full charge of assigned residents and staff, ensuring the nursing Team works together to complete their care delivery tasks. At shifts end, turns incomplete tasks over to the next shift. Documents task follow up requests on the 24-hour report.
• Makes daily resident visits to observe and evaluate the physical and emotional status of each resident.
• Regularly reviews comprehensive care plans for and with residents to ensure current delivery of care.
• Administers medications and treatments as ordered by the physician, compliant with facility standards and local, state and federal regulations.
• Administers emergency aid as required and maintains all emergency equipment.
• Actively participates in the QAPI activities for the Department of Nursing as assigned.
• When assigned, effectively participates in resident care conferences. Updates the residents CCP when changes occur through the writing and maintaining of the nursing portion of care plans.
• Develops and updates each residents CNA care assignment and ensures that it matches the CCP at all times.
• Transcribes and notes physician orders as required by facility protocols.
• Implements all documentation and report requirements inclusive of Medicare Parts A & B, Managed Medicare, and Title 19 according to the established policies and procedures of the facility and consistent with local, state and federal regulations.
• Implements all mandates for Change of Condition inclusive of nurse data collection, assessment documentation and notification to supervisor, physician and the family.
• Assists the Unit nurse manager/RN supervisor with oversight and direction of CNA work on each unit.
• Provides CNA staff on-going clinical updates for any residents especially for those that are new to their assignment or have experienced care delivery changes.
• Makes appropriate recommendations for personnel related action such as promotions, transfers or reassignments.
• Assigns and delegates the work of all staff allocated to the unit.
• Provides guidance and on-going training to co-workers as needed.
• Uses initiative, judgment and problem solving abilities in regards to residents, families and staff.
