Registered Nurse, Operational Excellence

The Operational Excellence Nurse provides holistic care to patients with kidney disease. As a National Float Pool employee, the position encompasses the administrator, Clinical Coordinator, charge Nurse or Registered Nurse role dependent on need. The position performs patient care and other duties assigned including supervision of Patient Care Tecnicians. The position reports to the director of operations and/or the first line in the facility/regional management team which may be a charge Nurse, Clinical Coordinator, administrator, area director regional director, or Vice President of operations dependent upon region/demographics.

Essential Duties and Responsibilities includes the following, and other duties and tasks as assigned.

• Develop and implement processes for program growth in accordance with Company goals.
• Plan/coordinate patient scheduling to assure timely acceptance of patients and effective staffing levels.
• Implement clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments.
• Demonstrate effective use of staff labor hours and facility supplies.
• Perform duties as assigned to meet the patient care or operational needs of the clinic.

• Review all incident reports; make recommendations and take action relative to incidents as appropriate; report at monthly QAPI meeting.
• Promote/Assist in quality management program through education and involvement of staff and patients in outcomes to achieve corporate goals for quality initiatives. Leads/Assists with, QAPI meetings and quality improvement committees.
• Coordinate/Assist in the functions of all departments and develop standards and methods of measuring patient care services, including a chronological record of services provided to patients as required by the ESRD Network Coordinating Council and Medicare.
• Complete audits to ensure EMR maintains chronological, thorough, and appropriate documentation in the patient record of all treatments, activities, and communication with the patient, physician and other healthcare professionals.
• Achieve/Assist in program's target goals for patient outcomes in accordance with quality patient care and Company goals.
• Complete and document monthly review of patient medication profiles. Administer medications as ordered by the physician
• Assist with the implementation of anemia management, medication protocols and all organizational quality initiatives as requested.
• Provide patient/staff education and provide referrals for further guidance as needed.
• Knowledge of and remain current with federal, state, local laws and regulations, including health care professionals practice act requirements.
• Assist/Develop, in implement and follow up necessary Corrective Action Plans for internal and external surveys.
• May assume Charge Nurse, Clinic Coordinatore or Facility Administrator responsibilities as needed.
• May fulfill responsibility of facility CEO as delegated by Governing Body.
• Conduct and/or participate in both formal and informal meetings with the governing body, Regional Director, Medical Director and the staff. Assure that appropriate staff meetings, in-service education meetings, and team patient care planning meetings are held monthly.
• Assure that Quality Assessment & Performance Improvement Program is current at all times.
• Perform duties at all times within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations.
• Complete patient admission process as outlined in policy and procedure, collaborating with team members regarding real or potential problems or needs relative to the treatment provided.
• Complete pre and post treatment physical assessments.
• Initiate and terminate patient treatments to include connecting and disconnecting catheters and cleaning catheter sites per protocol.
• Set up dialysis machines; report machine problems to biomedical technician and/or clinic's Administrator as appropriate.
• Clean, disinfect and provide quality control of dialysis machines as required by policy and procedures.
• Participate in infection control monitoring, implementation, and recording as requested.
• Use personal protective equipment as necessary.
• Be familiar with emergency equipment and all emergency operational procedures.
