Recovery Services Treatment Coordinator

SUMMARY
RS EDEN’s Recovery Services Treatment Coordinator directly serves clients in the Substance Use Disorder program, assessing immediate needs and executing tasks to address those needs. In collaboration with the treatment team and client, and client support system, the Treatment Coordinator assists in obtaining basic needs, securing benefits/insurance, scheduling and coordinating appointments for health, mental health, community services, housing, and other needs. The Treatment Coordinator is a liaison between the Admissions team, Clinical team, community, and interagency services and resources. This position requires confidence, creativity, and an ability to exercise individual discretion or judgment. Sound organizational skills and ability to multitask are a must. They must be able to devote the time and energy required to ensuring positive and customer service driven client interactions. They must possess a high level of crisis tolerance, and the ability to work through situations in a professional, therapeutic, and positive manner.
Pay ranges from $23-$25 per hour
RESPONSIBILITIES
- Conduct and/or review the Initial Services Plan and stabilization checklist for all new clients entering the RS EDEN Recovery Services programs
- Create a plan to meet client needs based on needs expressed by the client and identified on the Initial Services Plan and stabilization checklist
- Assist clients in obtaining basic needs to reduce stressors and barriers to engaging in treatment services
- Contact client supports and collaterals to understand expectations, case plans, court dates, and other relevant goals
- Meet with all clients at least biweekly to check in regarding treatment coordination needs
- In coordination with Health Services, Clinical team, Resiliency team, and others, schedule appointments and services on behalf of the client
- Inform clients of upcoming services and ensure they have means of access (transportation, need for peer support, coaching, etc)
- Manage electronic health record calendars corresponding to client appointments.
- Assign staff or recovery peers to assist clients in meeting treatment coordination needs and appointments
- In a timely manner, communicate client treatment coordination needs and progress with the multidisciplinary team
- Document and bill all services and client interactions in the clinical record in a timely and professional manner
- Support engagement in recovery treatment services by making concerted effort to schedule client appointments and activities outside of dedicated group/treatment service times
- Develop, foster and maintain relationships with interagency and community resources to support client needs
- Maintain contacts and attend meetings with collaborating agencies toward meeting client goals and progress
- Assist the Admissions team with client intake and orientation process including client education on program rules, expectations, program schedule and services
- In concert with the Senior Counselors and Clinical Supervisor, report out on billable services productivity
- Other duties as assigned
Qualifications/Requirements:
- Current Minnesota LADC/ADC-T or
- If no LADC/ADC-T, must meet the following:
- A bachelor’s degree in behavioral sciences or related field or
- Current certification as an alcohol and drug counselor, level I, by the Upper Midwest Indian Council on Addictive Disorders
- Skilled in the process of identifying and assessing a wide range of client needs
- Knowledgeable about local community resources and how to use resource to the benefit of the client
- Successful completion of 30 hours of classroom instruction on treatment coordination for substance use disorders
- At least 2,000 hours of supervised experience working with individuals with substance use disorder
- Excellent oral and written communication skills.
- Ability to relate to people from diverse backgrounds with multiple individual and systemic barriers to success
- Familiar with Microsoft Office applications
- Experience using electronic health record systems
- Current Tuberculosis screening
- Must be able to favorably pass applicable criminal history/background checks.
- Two years free of any problems associated with chemical dependency. Employees directly responsible for persons being served by the program are prohibited from abusing prescription medication or being in any manner under the influence of a chemical that impairs the individual’s ability to provide services or care.
