Description

PURPOSE OF POSITION:

The primary responsibility of the staff pharmacist is to ensure the accurate, age appropriate and timely preparation of all medication orders. The pharmacist is responsible for the supervision of pharmacy technicians, interns and students. The pharmacist facilitates the day to day operations of the pharmacy. The pharmacist will perform specific clinical functions in accordance with written policies and procedures. The pharmacist will provide drug information/education to patients, medical and hospital staff, as required.  This position reports to the Director of Pharmacy and may be required to assume the essential responsibilities and perform the duties of the Director in his or her absence.

 

RESPONSIBILITIES:

 

General Duties

  1. Receives, interprets and accurately dispense medications, monitoring for correct dosage, drug interactions, allergies and the appropriateness of therapy.  (Monitored through observation, QA and incident reporting.) 
  2. Prepares or oversees the preparation (calculates correct doses, ensures compatibility and correct concentration) of all IV fluids, including hyperalimentation solutions, chemotherapy, large volume parenterals, and IV piggybacks observing aseptic technique.  Must have documentation of sterile products yearly validation and required CE.
  3. Accurately prepares and dispenses all Controlled Substances according to pharmacy policies and procedures while maintaining all proper records. Is able to properly research and resolve outstanding discrepancies in the pharmacy and assist other units upon request.
  4. Supervises support personnel and assists in the training and development of all new pharmacy personnel.
  5. Provides accurate and complete drug information to other health care professional.
  6. Communicates with physicians concerning alternative therapy when non-formulary drugs are prescribed.
  7. Processes STAT/NOW medication orders completely in a timely manner.  
  8. Provides for the educational needs of pharmacy staff and other health care professional.  Promotes/provides inservice education as required.
  9. Accurately charges and credits medications to the patient’s bill. Audits daily medication charges and makes corrections as required.
  10. Monitors and documents adverse drug reactions.  
  11. Orders pharmaceuticals and processes invoices as required.
  12. Reviews Antibiotic/Microbiology information and makes appropriate recommendations regarding antibiotic therapy.
  13. Participates in the assigned Medication Usage Evaluations or special projects as assigned by the Director.
  14. Correctly monitors and makes recommendations on a patient receiving Total Parenteral Nutrition.
  15. Correctly monitors, calculates drug dosing (safe dose check), and provides patient education resources to patients receiving chemotherapy.
  16. Responsible for implementing and following all Pharmacy DNV standards.
