Job Description

The Social Worker (MSW) reports to the Social Work Manager of the Institute of Psychiatry. Under clinical practice guidance of the Social Work Manager, the Social Worker assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes a psychosocial assessment on the patient and caregivers throughout the hospitalization and arranges outpatient follow-up as needed. This position provides a wide variety of direct clinical social worker services in an acute care psychiatric setting for a complex caseload of patients including psychosocial assessment, active treatment (group therapy), discharge planning, and case management. The Social Worker identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

Position will be assigned to a treatment team on the General Adult Unit (3N) at the IOP and work with adults 18+ experiencing acute psychiatric crises. The social worker in this role will be responsible for assessment, group therapies and ongoing discharge planning to ensure a smooth transition to appropriate levels of care.  Shift is Monday through Friday 8a-4:30p with occasional cross coverage to assist on other inpatient units as needed.

Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.

Required Licensure, Certifications, Registrations : LMSW licensure by the South Carolina Board of Social Work Examiners required at the time of hire.

Physical Requirements:

(C) Ability to perform fine manipulation with fingers and hands. (C) Ability to reach overhead. (F) Ability to perform repetitive motions with hands/wrists/elbows and shoulders. (C) Ability to fully use both legs. (C) Ability to use lower extremities for balance and coordination.  (F) Ability to reach in all directions. (C) Ability to lift and carry 50 lbs. unassisted. (I) Ability to lift/lower objects 50 lbs. from/to floor from/to 36 inches unassisted. (I) Ability to lift from 36” to overhead 25 lbs. (I) Ability to exert up to 50 lbs. of force. (F) Examples include: To transfer a 100 lb. patient that can not assist in the transfer requires 50 lbs. of force. For every 100 additional pounds, assistance will be required from another healthcare worker. 20 lbs. of force is needed to push a 400 lb. patient in a wheelchair on carpet. 25 lbs. of force is required to push a stretcher with a patient with one hand. Ability to maintain vision 20/40 corrected. (C) Ability to see and recognize objects close at hand or at a distance. (C) Ability to match or discriminate between colors. (C) Ability to determine distance/relationship between objects; depth perception. (C) Good peripheral vision capabilities. (C) Ability to maintain hearing acuity, with correction. (C) Ability to perform gross motor functions with frequent fine motor movements. (C) Ability to deal effectively with stressful situations. (C) Ability to work rotating shifts. (F) Ability to work overtime
