Description:

  • The Employee agrees to provide APRN service onsite at the Health Department to perform clinical services weekly. These Dates are to be predetermined and agreed upon by both parties.
  • Hours for these services will be between to the hours of 7:30am to 5:30pm. Depending on availability.
  • Contractor agrees not to bill the patient for any services rendered. The contract is payment in full.
  • The Health Department is responsible for billing all third parties and self-pay patients.
  • The Employee agrees to maintain a valid APRN License thru the Kentucky Board of Nursing and is responsible for continuing education credit per license.
  • The Employee agrees to abide by the rules and regulations regarding confidentiality of personal medical records as mandated by Health Insurance Portability and Accountability Act (42USC 1320d) and set forth in federal regulation at 45 CFR Parts 160 and 164.
  • Employee will provide APRN services for the Health Department to support the agreement with University of Kentucky’s RAPID Grant Study (Rapid Access to PrEP through Integrated care Delivery) protocol in Madison County, Kentucky.  The NIH/NIDA funded study is optimizing HIV Pre-Exposure Prophylaxis (PrEP) care delivery through adaptive intervention strategies for underserved rural people who inject drugs.

  • The Employee will provide APRN services for the University of Kentucky’s RAPID study, as outlined in the study’s protocol.  This includes PrEP clinical assessment and care, including testing for HIV, Syphilis, Hepatitis B, Creatine Clearance and Pregnancy at the initial visit. Participants who initiate PrEP will also receive follow-up testing at three and six months with an addition of PrEP adherence testing for Tenofovir disoproxil fumarate (TDF). The Employee may be required to attend meetings with the Health Department and University of Kentucky when requested.
